Aftercare Program
An aftercare program helps facilitate a seamless transition from inpatient treatment to independent, substance-free living, while providing additional support whenever necessary. There are several different components of aftercare, all which play an integral role in the overall aftercare process.
We face many challenges after leaving treatment, and a strong aftercare program will help you meet those challenges without resorting back to substance abuse. It’s easy to fall back into your old habits and end up right back where you started. Our recovery services offer tools and guidance to avoid any potential pitfalls in your sobriety.

Why Do People Relapse?
It can be difficult to stay sober after leaving treatment because of a number of factors. First, there is often a sense of isolation after treatment. This can be due to a loss of support from family and friends who may not understand the challenges of maintaining sobriety.
Additionally, many people who have been through treatment may have difficulty finding employment or stable housing, which can make it difficult to stay sober. Finally, relapse is often a part of recovery, and people who have relapsed may find it difficult to stay sober after treatment.
You Can Stay Sober After Leaving Treatment
Medical detox and inpatient treatment are only the very first steps on the lifelong journey of addiction recovery — the recovery process does not stop simply because you have completed treatment. The goal of initial care is to physically stabilize clients while preparing them for an intensive period of therapeutic submersion — inpatient rehabilitation.
Once residential treatment is complete, an individual will advance in her or her recovery journey, transitioning to aftercare. There are many aftercare treatment options when it comes to continuous care. Your team of treatment advisors and your individual counselor will help you develop a detailed aftercare plan based on your personal background and prior treatment experience.
Aftercare will vary depending on your individualized needs. It is important to note that the time directly after inpatient treatment and before a full transition into independent living is crucial; this is the period of time during which those new to sobriety tend to relapse if they do not have a solid aftercare program in place.
If you're ready to start your journey to recovery, we're here to help.
Sober Living Residences
Sober living housing is a large step down from residential treatment, and a small step above completely independent living. Those who are new to sobriety often benefit from living in a safe, structured and supportive environment as they begin to navigate day-to-day life.
Sober living housing enforces a structured set of rules. These rules are all geared toward helping recent graduates of inpatient treatment develop a solid routine that will ultimately lend itself toward long-term, drug and alcohol-free living.
Some of the guidelines that will be enforced include:
- Staying completely sober (regular drug tests will be administered)
- Keeping personal space and common areas clean
- Finding a job and working full-time
- Becoming financially independent
- Respecting others in the household
- Participating in 12-step meetings on a daily basis
- Waking up/going to bed at a certain time
Curfew will also be enforced. This structured living allows the development of crucial life and social skills, while providing additional support during a potentially difficult time.
Continued Therapeutic Care
Group and individual therapy sessions will likely continue throughout aftercare. Group sessions will be part of an outpatient program; outpatient treatment is a continuum of care, one significantly less intensive than inpatient, but one that still provides the same services. Outpatient treatment will be held several times per week for several hours (usually between three and five).
In group counseling sessions, an individual will share personal experiences and listen to the experiences of others, fostering healthy relationships with like-minded people and developing crucial life skills and coping skills. In many cases, individual counseling sessions will be offered as part of the outpatient program.
Support Groups
Ongoing involvement in 12-step support groups will also be a mandatory part of aftercare. Fellowship programs like Alcoholics Anonymous, Narcotics Anonymous and Al-Anon have been proven effective in helping newly sober individuals achieve long-term recovery. At Evoke Wellness at Cohasset, we incorporate mandatory 12-step meetings into every stage of our recovery program, including an aftercare program.
